Holistic Health Coaching Consent

 

By signing up for my services, you acknowledge that:

• Holistic Health Coaching encourages self-empowerment and self-responsibility.

• It does not cure, diagnose, prescribe medicine, or medically treat conditions. Seek further advice for health concerns.

• Temporary changes in emotions, moods, thoughts, or physical sensations may occur. Seek further advice if necessary.

• Home practices will be recommended to support your sessions. Commitment to these practices is required.

• Referrals to other professionals may be made if necessary.

• Confidentiality is maintained unless permission to share is granted.

Client Responsibilities & Code of Conduct

 

Clients are expected to:

• Commit to the program to achieve the best results.

• Follow the advice and home practices recommended.

• Communicate openly and honestly during sessions.

• Respect the confidentiality of the sessions.

• Notify me at least 24 hours in advance if you need to reschedule an appointment.

 

Fees Policy

• Payments must be made within 24 hours of invoicing.

• Late payments may result in appointment cancellation and a pause in services.

• Consultations are not covered by Private Health Insurance or Medicare.

 

Cancellation Policy

• Provide at least 24 hours’ notice to change appointments.

• Less than 24 hours’ notice may result in the loss of that appointment.

 

Booking and Payment Policy

 

Payment Options

• Full Payment: Pay the full amount at the time of booking.

• Half and Half: Pay 50% at the time of booking and the remaining 50% halfway through the program.

• Monthly Installments: Pay in monthly installments based on the program duration. The first installment is due at the time of booking.

• Credit/Debit Card through the Wix booking system

• Bank Transfer

 

Payment Timeline

• Full Payment: Due at the time of booking.

• Half and Half: First half due at the time of booking, second half due halfway through the program.

• Monthly Installments: First installment due at the time of booking, subsequent payments due on the same day of each month.

 

Late Payment Policy

• Reminder emails are sent 48 hours before the due date.

• A $10 late fee is added if payment is not made within 24 hours of the reminder.

• If payment is not made within 2 business days, the program will be paused until payment is received.

 

Cancellation and Refund Policy

You may cancel the program after 6 weeks if unsatisfied, provided you can show evidence of following the given advice. Refunds are calculated based on the remaining paid months not completed.

Email reminders are sent 48 hours before each payment due date. Contact me via WhatsApp, Instagram, email, or the website contact page for any inquiries.

Booking Appointments

Initial appointments can be booked on the website or during a free 30-minute phone call. Follow-up appointments are booked at the end of each session, with confirmation and reminder emails sent.

  • I am not liable for any direct, indirect, incidental, or consequential damages resulting from the use or inability to use my services. By using my services, you acknowledge that you do so at your own risk and that my liability is limited to the maximum extent permitted by law.

Intellectual Property Rights

All content provided during the services, including but not limited to written materials, videos, and audio recordings, remain the property of Holistic Wellness With Charlotte. Clients may use these materials for personal use only and may not distribute or reproduce them without written permission.

 

Governing Law and Dispute Resolution

These terms are governed by the laws of Australia. Any disputes arising under or in connection with these terms shall be resolved through mediation or arbitration in Australia.

 

Modifications to Terms

Holistic Wellness With Charlotte reserves the right to modify these Terms of Service at any time. Clients will be notified of any changes via email or through an announcement on the website. Continued use of the services constitutes acceptance of the revised terms.

 

Dispute Resolution Process

In the event of a dispute, clients are encouraged to first contact me to resolve the issue amicably. If a resolution cannot be reached, the following process will apply:

1. Mediation: Both parties agree to engage in mediation with a neutral third party to resolve the dispute.

2. Arbitration: If mediation fails, the dispute will be settled by arbitration in accordance with the rules of the Australian Centre for International Commercial Arbitration (ACICA).

3. Legal Action: If arbitration is not successful, either party may pursue legal action in the courts of Australia.
